New Patient Information
If after visiting our website, you would like to contact us about becoming a new patient, we welcome your call. We look forward to meeting you and giving you the best possible care to help you acheive optimum dental health, as well as developing a long-term relationship with you and your family. If you would like to save some time, you can download our New Patient Forms (see below). You can either fill out the paperwork and bring it to your first appointment or arrive 15 minutes prior to your appointment and we will assist you in filling out the necessary information. We work with many dental insurance plans. However, we recommend that you contact your insurance provider to verify what specific plan you are enrolled in with your company. Then, please review our insurance section for more details or feel free to call the office to verify that we participate with your specific insurance plan. Be sure to bring your insurance information, including your card, to your dental visit. We understand that you are probably coming from another dentist, therefore if you have had x-rays taken within the last two years, we ask that you request these x-rays be sent to you and that you bring them with you to your appointment with us. Some x-rays need to be retaken, but we will explain what will be necessary for your specific situation at your appointment. Please call the office if you require a premedication prior to your dental visits. Many of the premedication guidelines has recently changed, so please don't hesitate to contact us with questions regarding this.
